If you are a caseworker, counselor, or social service provider, you can refer clients to our family resource navigators. We’ll follow up with them, offering information and resources.

When should I refer a family to the SEAS Program?

Social service providers can refer any family to SEAS for support around connecting with intervention programs, therapy services, school programs, mental and behavioral healthcare, and basic needs. Our family resources navigators are experts in local programs and services, and will assist families in getting the support they want and need.

Family Resource Navigators can support the families you refer by:

  • Enrolling children ages 0-3 with concerns about developmental delays into our local early intervention program, Early Support for Infants and Toddlers (ESIT)

  • Providing guidance to families looking to get Individualized Education Program (IEP) evaluations with their local school district

  • Providing information about how to get children onto local waitlists for speech, occupational, and physical therapies

  • Facilitating referrals to the GIDES process, including an autism evaluation, with a medical provider’s referral

  • Assisting families in finding options for mental and behavioral healthcare

  • Offering resources to support children with disabilities

  • Supporting families in finding other supportive resources, such as social connection groups and guidance towards basic needs supports

Family resource navigators cannot:

  • Evaluate or diagnose children

  • Make medical recommendations

  • Provide case management

  • Complete forms for families

  • Help families obtain an aid for a child in childcare

After a referral is received, family resource navigators will:

  1. Call the family

  2. Ask and listen about the concerns they have for their children

  3. Provide information about resources and make program referrals

  4. Follow up to see if they were able to connect to the suggested resources

  5. Fax you a report to let you know the outcome of our connection
